Under direct supervision, this role is responsible for the day-to-day filing duties. Team member will, at all times, respond to patients, visitors and co-workers in a pleasant, helpful and professional manner.

1. Receive and respond to requests for medical record information from patients, attorneys, insurance

companies and specialties via fax, phone or in person; log and track all requests and update log as

appropriate.

2. Prepares requests for submission to Medical Record Release company when appropriate per policy.

3. Prior to release verifies accuracy of records being released. Release requested records according to

HIPAA statues, state laws, UMMS (corporate) and departmental policies and procedures regarding

disclosure of patient protected health information as requested in either electronic (fax, CD, etc) or paper

format.

4. Be proficient with receipt of correspondence via the electronic fax server (Fax batches) and proper

distribution of all such correspondence into the Electronic Health Record (EHR) and to providers

according to Harford Primary Care (HPC) and/or UMMS policies and procedures.

5. Receive and properly scan and distribute mail and other paper correspondence or records into the

EHR and providers according to HPC policies and procedures.

6. Properly document release of protected health information in the EHR, including fully utilizing the

"Quick Disclosure" in the electronic chart.

7. Route to appropriate clinical staff the results of testing such as Mammography, Colonoscopy and

other Quality Measures according to UMMS UCH and HPC policies and procedures.

8. Enter dates of completed tests ordered by HPC providers on either lab logs or by completing

appropriate sections in EHR.

9. Ensure compliance with HIPAA statues, state laws, UMMS (corporate) and departmental policies and

procedures with regard to confidentiality of patient's protected health information.

10. Retrieve reports from labs, radiology providers or specialists as required by providers or clinical staff.

11. Maintain off-site charts and data requesting for return to the office as needed.

12. Provide coverage of Medical Record duties for other HPC offices as needed.

13. Perform a variety of routine and non-routine clerical duties, including the operations of office

equipment.

14. Performs other duties as assigned.
