Job Summary
The Medical Records Clerk is responsible for collecting, maintaining, and making available to authorized users timely, accurate and complete patient health information.
The Medical Records Clerk:
Responds to chart requests quickly, asking coworkers' for assistance when needed
Retrieves patient charts and re-files charts in proper sequence
Completes requests for information from patients, physicians, attorneys, and insurance companies following departmental policies to safeguard patient confidentiality; maintains a log of this activity within the patient's chart
Locates records that have been checked out or are missing, in accordance with departmental policies for safeguarding patient records
Follows established departmental policies, procedures, continuous quality improvement objectives, safety, environmental and/or infection control standards
Scans/indexes external documents appropriately into patient charts with correct date, provider, and document type for ease of retrieval by medical staff within 48 hrs of receipt
Follows up on requests for information from other institutions that have not been received in a timely fashion
Works with supervisor to assist in audits and chart reviews for providers
Performs other related duties as assigned.

Required Skills:
Qualifications
Must possess a High School Diploma or higher
Minimum two years recent experience in a medical office setting
A working knowledge of medical terminology
Minimum 1-2 years of hospital or medical records experience required.
Ability to use time wisely in preparing work area to meet high-paced demand.
Must be detail oriented.
Must be able to communicate verbally and non-verbally in a professional manner.
Must be able to adhere to hospital financial and admitting policies.
Ability to promote positive relationships with patients and staff.
Must have problem solving and decision making skills.
Must demonstrate excellent phone etiquette and exceptional customer service skills
