The Social Worker serves as a Veteran Directed Care (VDC) Program point of contact. VDC offers Veterans the opportunity to receive personalized in-home services and avoid placement into an institutional setting and remain at home through allowing the Veteran / Authorized Veteran Representative to develop a personalized Care Plan in coordination with the local Area Agency on Aging utilizing VA funds and receiving AAA assistance to budget for private hired care. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
